Information Clause on Processing of Personal Data

In accordance with Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) the University of Warsaw informs that:

  1. The Administrator of your personal data is the University of Warsaw 26/28 Krakowskie Przedmieście Street, 00-927 Warsaw, represented by the Rector.
  2. A Data Protection Officer has been appointed by the University of Warsaw who is based at 26/28 Krakowskie Przedmieście Street, 00-927 Warsaw, CIUW building, p. II, room nr 202. The Officer can be contacted via e-mail: iod@adm.uw.edu.pl or by phone – 22 55 22042.
  3. Your personal data will be processed for the purpose of the publication of the submitted academic paper on the basis of the agreement concluded with the Jagiellonian University Press and or the decision of the editor of the journal.
  4. Providing personal data is voluntary. However, it is obligatory for the acceptance of your paper publication The withdrawal of consent shall result in no possibility for your paper to be accepted for publication at the Jagiellonian Univertsity Press.
  5. Your personal data will be shared with: the publisher, academic reviewers and the publisher’s partners vital for the editorial process (e.g. editors, proofreaders), and, if relevant laws and regulations apply, to other parties with which the University of Warsaw Press has legal obligation to share the personal data (e.g. The Polish Social Insurance Institution, Tax Office, police, the District Attorney).
  6. Your personal data will be shared with third countries/international organizations on the basis of: agreements regarding academic databases such as ejournals.eu, Scopus, Web of Science, Ceeol, Ebsco, Proquest
  7. Your personal data will be retained for archiving purposes for the duration provided by the law (specifically regarding academic parametrization)
  8. You have the right to: access the data and demand its rectification, deletion, processing restrictions, transfer the data, object to the processing of data, withdraw your consent at any time in cases and under the conditions stipulated in “General Regulations”.
  9. You have the right to withdraw your consent at any time regardless of the compliance of the sharing of the data with the law. The withdrawal of consent may be sent by email to bookhistory@uw.edu.pl, via regular mail at Studies into the History of the Book and Book Collections, Nowy Świat 69, 00-927 Warszawa, or in person at Studies into the History of the Book and Book Collections, Nowy Świat 69, 00-927 Warszawa, p. 3, room 158, 139. The withdrawal of consent shall result in restriction or the lack of publication or parametrization of the academic paper.
  10. You have the right to file a complaint with The President, Personal Data Protection Office (UODO) if you consider the processing of your personal data to be in violation of the provisions of the “General Regulations”. I hereby confirm that I have read and understood the above information.
